There’s a particular kind of exhaustion that sets in when you spend enough time reviewing books about living longer. The genre has become a circus. On one end, you have the biohackers — men with way too much venture capital and a pathological terror of dying — and on the other, a fog of wellness mysticism so thick you can practically smell the bone broth. I’ve read enough of both varieties to know the tells within the first few pages. So when Eric Topol’s Super Agers: An Evidence-Based Approach to Longevity landed on my desk, I’ll admit I approached it with the specific wariness of someone who’s been fooled before.
I wasn’t fooled. That’s the short version.
Topol, a cardiologist and one of the more respected voices in digital medicine, opens with a distinction that sounds simple but carries real weight: lifespan versus health span. The first is just years. The second is years you actually want. His book is essentially a cartography of the biomedical revolution now converging around us — genomic sequencing, AI diagnostics, GLP-1 drugs, cellular engineering — and an argument that these tools, used wisely, could radically compress the period of chronic suffering that swallows so many people’s final decades. He organizes all this around five domains he calls lifestyle+ (expanded, notably, to include environmental toxins and social isolation), cells, omics, AI, and drugs and vaccines. It’s a tidy framework for an enormously untidy subject.

Now, I’ll be honest about what this book is not. It is not an easy read. Topol writes with the cadence of a physician who has spent thirty years delivering complicated news to frightened patients — precise, slightly brisk, unbothered by elegance. There are stretches where the data piles up with genuine ferocity: hazard ratios, polygenic risk scores, randomized trial citations stacked three deep. He anticipates the complaint himself, pausing at one point to essentially ask the reader to trust him through the density. I was tempted, at first, to read this as the classic failure of the brilliant specialist — someone who knows too much to translate it compellingly. But I kept reading, and somewhere around the middle I changed my mind. The relentlessness starts to feel intentional. By refusing to simplify, by declining to hand you a silver bullet wrapped in inspirational language, Topol is doing something almost countercultural in this genre: he’s making you sit with how slow and unglamorous real medical progress actually is. The density isn’t a flaw. It’s a kind of inoculation against the pseudoscience he’s clearly furious about.
Where the book gets genuinely complicated — and, to my mind, genuinely interesting — is when Topol’s techno-optimism runs headlong into the American healthcare system. He is an enthusiast, and an infectious one. He describes a near future where AI reads your retinal scan and predicts Alzheimer’s decades before symptoms emerge, where CRISPR quietly edits away genetic liabilities before they metastasize into catastrophe. It’s a breathtaking vision. It is also, and I couldn’t help but keep returning to this, a vision being articulated in a country that routinely fails to get basic insulin to diabetic patients who can’t afford it. Topol isn’t blind to the tension — he calls out microplastics, forever chemicals, the disproportionate environmental burden borne by marginalized communities, and even states plainly that if extended health span becomes a luxury reserved for the wealthy, the entire enterprise should be considered a failure. That sentence hit me. And then the book more or less moved on.
That’s the lingering frustration. The equity problem gets acknowledged with genuine feeling and then mostly set aside, while the text returns, page after page, to interventions that currently cost hundreds of thousands of dollars per patient. His answer to the access crisis — vague invocations of public-private partnerships — lands with a kind of helpless shrug that feels wildly disproportionate to the precision he applies everywhere else. He maps the atomic architecture of an mRNA nanoparticle with meticulous care, then gestures vaguely at systemic greed as though it were weather. The biological pathology is treated. The societal one is left entirely on the table.

What you’re left with, finally, is a book of genuine and sometimes stunning scientific literacy that functions, almost against its own intentions, as a mirror held up to our fractured medical moment. Topol has done the hard work of proving, with real rigor, that the tools to extend healthy human life are no longer science fiction. That’s not a small thing. But the emotion that lingers after the last page — and this is what I keep coming back to — isn’t hope exactly. It’s something quieter and darker: the creeping recognition that we’ve figured out how to delay our own decay, and that we are almost certainly going to ration it along the same cruel lines we’ve rationed everything else.
